The researchers asked staff and decision-makers at 13 Philadelphia HIV clinics about using community health workers to deliver a medication-adherence program. They wanted to understand how patients could be identified and referred and how the program could fit into clinic teams.

Plain Language Abstract

Managed Problem Solving is an evidence-based program that can support HIV medication adherence and viral suppression, but community clinics do not widely use it. Researchers interviewed stakeholders from 13 Ryan White-funded clinics in Philadelphia, including prescribing clinicians, other clinical staff, administrators, and policymakers. The interviews examined barriers and facilitators to having community health workers deliver the program. Stakeholders were generally receptive. They emphasized the need for workable methods to identify and refer patients and for strong communication and integration between community health workers and the broader care team. Researchers used a rapid qualitative process to organize these determinants along an implementation pathway so that they could guide strategy development. The study shows how clinic perspectives can be incorporated before rollout of an evidence-based behavioral intervention. Because it was a preimplementation qualitative study, it reports stakeholder expectations rather than measured adoption or effectiveness. It did not test whether community health worker delivery increased medication adherence, retention, or viral suppression in participating clinics.
