The researchers talked with rural Alabama residents and people involved in HIV and community health. They wanted to understand local testing barriers, community concerns, and practical opportunities for designing a mobile HIV-testing service.

Plain Language Abstract

Rural Alabama has substantial gaps in HIV diagnosis and testing, especially among Black residents facing geographic and economic barriers. Researchers interviewed 22 stakeholders involved in HIV or community health and 10 adults living in rural communities. They used rapid qualitative analysis and discussed findings with community partners to guide a future mobile testing service. Participants described how racism, poverty, rural location, and cultural norms limited health care access. They also reported limited sex education, low HIV knowledge, inaccurate perceptions of risk, and continuing stigma. The message that people with an undetectable viral load do not sexually transmit HIV was not well understood. Participants believed community involvement could strengthen trust and communication, and they viewed new testing approaches as acceptable ways to reduce barriers. The authors concluded that working with trusted community “gatekeepers,” particularly faith leaders, may help introduce new services. These findings represent the perspectives of 32 interviewees and are not population-wide estimates. The study informed implementation planning; it did not test whether mobile testing increased testing or diagnoses.
