Plain Language Abstract

Mississippi has low uptake of pre-exposure prophylaxis, or PrEP, relative to the number of new HIV diagnoses. Researchers analyzed records from Open Arms Healthcare Center, the state’s largest PrEP provider, covering 2017 through mid-2020. Staff offered PrEP during 680 of 721 eligible encounters, or 94%, representing 526 unique people. Patients accepted an offer—meaning they agreed to a clinical PrEP evaluation—during 58% of encounters. Acceptance varied substantially: it was lowest among transgender and nonbinary patients at 15.8% and highest among people reporting a partner living with HIV at 93.3%. The analysis shows how offers and acceptance differed in this clinic but does not explain all reasons for decisions or show how many people ultimately started and continued PrEP. Because encounters from the same individuals may recur, encounter-level percentages are not the same as person-level uptake. The authors concluded that the clinic reached a highly affected population but needs targeted work to improve acceptance, especially among transgender and nonbinary patients.
