The researchers interviewed pregnant patients with a recent sexually transmitted infection about HIV-prevention medication. They wanted to understand perceived HIV risk, knowledge and concerns about PrEP, the role of obstetric clinicians, and preferences for different ways of taking it.

Plain Language Abstract

Pregnancy can be a period of HIV vulnerability, but little is known about how pregnant patients view pre-exposure prophylaxis, or PrEP. Researchers interviewed 20 pregnant patients with a recent sexually transmitted infection at a US urban obstetrics clinic. Their median age was 24; 95% were African American and 70% reported an unplanned pregnancy. Despite the recent infection, most viewed their HIV risk as low or absent and knew little about PrEP. Concerns about medication safety and low perceived risk reduced acceptability. Very few had discussed PrEP with an obstetrician-gynecologist, but those who had generally viewed it favorably. Participants wanted to choose among formulations based on ease of use, personal acceptability, and previous experience with medications. The study suggests obstetric clinicians may have an important role in providing information and discussing options tailored to patient preferences. Because this was a qualitative study of 20 patients at one clinic, the findings are not population-wide estimates and do not show that counseling would necessarily increase uptake or adherence.
