The researchers examined how New York City HIV organizations were trying to reduce stigma. They wanted to understand which activities were being used, how well those activities were supported by evidence, and what helped or hindered organizations from carrying them out.

Plain Language Abstract

HIV-related stigma can prevent people from seeking prevention, testing, and treatment services. Researchers surveyed and interviewed a convenience sample of 27 New York City organizations providing HIV prevention or treatment services between March and August 2020. They mapped the organizations’ stigma-reduction activities, considered the supporting evidence, and examined barriers and facilitators at several levels. Organizations described integrated health services, staff who represented the communities served, and training as important supports. They most often addressed overlapping stigmas by combining HIV services with mental health and substance use care. Barriers occurred at several levels, with organizational structure and limited capacity described as especially difficult. The organizations were already using many stigma-reduction activities, but explicit intersectional frameworks and formal evaluation of whether activities reduced stigma were uncommon. Because this was a mixed-methods assessment of a convenience sample, the findings describe participating organizations and do not establish that any activity caused better outcomes. The authors call for future studies that evaluate effectiveness while also studying implementation.
