Plain Language Abstract

Only a minority of young people who could benefit from pre-exposure prophylaxis, or PrEP, have prescriptions, and new formulations may address some barriers. Researchers surveyed 737 cisgender men who have sex with men, ages 15–24, across the United States. Participants compared six products in a structured choice experiment. Nineteen percent currently used daily PrEP and 9% had used it previously. Across options, participants prioritized effectiveness, absence of side effects, and cost. Daily pills ranked highest, followed by on-demand pills, injections, implants, antibody infusions, and rectal douches. Preferences varied by age, insurance, sexual behavior, prior PrEP use, testing history, and previous sexually transmitted infection. Participants choosing pills emphasized ease, while those choosing injections or implants felt more protected; those selecting infusions valued not forgetting doses. The study measures stated preferences, not actual adoption or persistence. Although next-generation products ranked lower overall, offering choices that address different concerns may support acceptability. Results do not show which products will increase population-level use.
