The researchers studied how a New York City program that reconnects people with HIV to care responded to start-up problems and how those responses helped the program and participating organizations change.

Plain Language Abstract

The Bottom Up Project uses health information exchange alerts and peer outreach workers to find and reconnect people with HIV who have fallen out of care. Researchers examined how the New York City partnership handled implementation problems during start-up. They interviewed six implementation staff and reviewed 35 sets of meeting minutes and protocols. Analysis identified 20 major challenges across the project’s seven steps, with two to five challenges per step. Problems occurred at several levels and ranged from ongoing difficulties to crises. Teams responded by absorbing stress while maintaining operations, making short-term adaptations, or creating larger structural changes. Short-term adaptation was most common. During severe stress, the project sometimes simplified activities while protecting core functions. The findings describe how one partnership responded to challenges; they do not test whether resilience strategies improved patient outcomes. The authors concluded that combining a standard implementation framework with a resilience lens can reveal the active processes through which programs and organizations adjust, mature, or struggle.
