The researchers surveyed Black men who have sex with men in Mississippi about where and how they would feel comfortable receiving HIV-prevention medication. They wanted to learn whether experiences of homophobia or racism were associated with those preferences.

Plain Language Abstract

Pre-exposure prophylaxis, or PrEP, is underused among Black men who have sex with men in the South, and experiences of discrimination may shape where care feels safe. Researchers assessed comfort with different PrEP locations and delivery methods among 65 Black men who have sex with men in Mississippi. Statistical tests examined associations with reported homophobia and racism. Participants reporting more homophobia were less comfortable receiving PrEP at academically affiliated clinics, community-based organizations, and sexually transmitted infection or HIV clinics. Participants reporting more racism were more comfortable receiving PrEP by mail. These findings suggest that private delivery options and attention to provider and organizational bias may matter when designing services. However, the study measured associations in a small sample and cannot show that discrimination experiences caused particular preferences. Comfort with a hypothetical or available delivery option does not necessarily predict actual PrEP initiation or persistence. The results should guide further exploration rather than be treated as representative of all Black men who have sex with men.
