The researchers wanted to understand why Black same gender-loving men in Mecklenburg County did or did not access HIV-prevention medicine. They worked with community and clinic partners to design strategies for easier access and prepare for later testing.

Plain Language Abstract

Use of pre-exposure prophylaxis, or PrEP, is low among Black same gender-loving men in Mecklenburg County, North Carolina. Researchers formed a coalition with community representatives, a PrEP provider, and researchers. They conducted four focus groups with men not using PrEP, 17 interviews with current or former users, and six readiness assessments with community organizations and clinics. Participants generally knew about PrEP but often believed it was unaffordable. Many did not know how to obtain it or about the county program offering free services to uninsured people. Scheduling barriers and mistrust of providers also limited access. The partnership developed client-level strategies to increase comfort and practical knowledge about obtaining PrEP and clinic-level strategies to change infrastructure and make access easier. Organizational assessments helped prepare sites for future piloting. This was an implementation-planning study: it identified determinants and developed strategies with community input but did not test whether those strategies were acceptable, increased PrEP uptake, or improved continued use. The qualitative findings reflect participating men and organizations and should not be treated as countywide prevalence estimates.
