The researchers studied sexual and gender minority people who could benefit from HIV-prevention medication. They wanted to learn which participants had discussed it with a clinician and what characteristics were linked with having that conversation.

Plain Language Abstract

Medical providers are important gatekeepers for pre-exposure prophylaxis, or PrEP, yet many people who could benefit never discuss it in clinical care. Researchers analyzed Together 5000, an internet-based US cohort of sexual and gender minority people aged 16–49 who were vulnerable to HIV. Among 6,264 participants eligible for PrEP uptake, they modeled factors associated with having discussed PrEP with a provider. Only 31% reported such a conversation. Among those who had spoken with a provider, 45% said they would start PrEP; willingness was more common among participants older than 24. The findings suggest that routine medical visits may offer opportunities to begin and continue conversations about HIV prevention rather than relying only on narrowly targeted outreach. However, this observational analysis identifies reported associations and cannot show that a provider discussion caused willingness to use PrEP. The internet-based cohort and self-reported measures may not represent all US sexual and gender minority people or confirm that participants subsequently received prescriptions.
