The researchers wanted to understand what helps or hinders clinics from offering rapid HIV treatment in Miami-Dade County and what shapes patients’ interest in it. They also identified strategies that could be tested later.

Plain Language Abstract

Starting HIV treatment soon after diagnosis supports earlier viral suppression, but rapid treatment is not implemented consistently. Researchers held focus groups with staff from four Miami-Dade County clinics and interviewed 31 recently diagnosed patients. Using an implementation framework, they examined influences involving the treatment model, external environment, clinic organization, staff, patients, and implementation process. Clinic participants emphasized treatment complexity and adaptability, communication and networks between clinics, patient needs rooted in structural inequities, leadership, resources, staff flexibility, patient navigators, and organizational champions. Patients similarly discussed treatment complexity and advantages, their needs and clinic networks, provider knowledge, warm and affirming interactions, and stronger outreach. These findings show that rapid treatment depends on connected factors at several levels and that clinic implementation and patient demand may require different but coordinated responses. The researchers used the findings to identify potential strategies for a future implementation trial; they did not test whether those strategies improve rapid treatment access or outcomes. Because the study was qualitative and involved four clinics and 31 patients, the themes reflect these participants’ perspectives rather than population-wide estimates.
