The researchers interviewed people who had implemented PrEP programs in different types of health care settings to learn what helped, what caused problems, and how organizations adapted.

Plain Language Abstract

Pre-exposure prophylaxis, or PrEP, has been integrated successfully in some clinical settings but not others. Working with the New York City health department and a planning group, researchers interviewed 20 people who had implemented PrEP programs in settings such as primary care, emergency departments, and sexual health clinics. The sample initially emphasized New York and later expanded nationally to include more program types. Participants discussed implementation conditions and strategies in closely connected, context-specific ways. Common issues included whether staff felt a strong need for change, whether PrEP fit existing services, its priority relative to other work, staff and stakeholder knowledge and beliefs, and the costs of implementation. These qualitative findings describe patterns across experienced programs but do not rank strategies or test which one caused better uptake. The authors concluded that formal implementation research should be connected more closely to how frontline implementers understand and carry out their work. Local context remains important when organizations choose and adapt strategies.
