The researchers studied Black cisgender women who received HIV-prevention medication at a Chicago health center. They wanted to learn how many continued treatment for six months and what clinical experiences, beliefs, and barriers were linked with continued use.

Plain Language Abstract

Black cisgender women experience disparities in HIV prevention, but limited information is available about continued use of pre-exposure prophylaxis, or PrEP. Researchers reviewed electronic health data for 112 Black cisgender women prescribed PrEP at a Chicago federally qualified health center. Persistence meant having medication available for at least 86% of days over six months. They also interviewed eight women selected according to whether they persisted. Only 18% met the persistence definition. In adjusted analyses, neighborhood, the reason for the visit when PrEP began, and the year treatment started were associated with persistence. These relationships do not show that those factors caused continued or discontinued use. Interview participants offered little evidence that cost or difficulty taking pills were major barriers. They instead described low community awareness, the importance of providers, stigma, side-effect concerns, pregnancy concerns, and decisions based on their own perceived HIV risk. The small qualitative sample reflects individual experiences. The findings identify real-world barriers and possible targets for future interventions rather than proving how to increase persistence.
