The researchers explored whether a mobile app could help women with opioid use disorder take both HIV prevention medication and treatment for opioid use, and what practical barriers might limit use.

Plain Language Abstract

Women with opioid use disorder rarely use pre-exposure prophylaxis, or PrEP, even when they could benefit. Researchers interviewed 20 women receiving services from a Philadelphia community organization between May 2021 and February 2022. Every participant had been prescribed reSET-O, an app that supports treatment for opioid use disorder. Participants’ average age was 37; half reported recent injection drug use, six were taking buprenorphine, mental health symptoms were common, and half described transactional sex. Most expressed strong interest in PrEP and believed the app could acceptably support both PrEP and opioid treatment adherence. However, only two participants redeemed the app prescription. Limited phone and internet access were common barriers. The contrast between favorable interview responses and very low actual uptake shows that stated acceptability did not translate into use. The authors concluded that obtaining and activating the app was a major implementation barrier and that practical access challenges must be addressed before this approach can support PrEP in this population.
