The researchers interviewed Alabama adolescents and caregivers separately to understand how families talk about sex and HIV prevention medication and whether caregiver involvement might help selected adolescents consider PrEP.

Plain Language Abstract

Adolescents experience a disproportionate burden of HIV, but family communication about sex and pre-exposure prophylaxis, or PrEP, can be difficult. Researchers separately interviewed nine adolescents aged 14–18 in Alabama and seven caregivers, forming five pairs and two groups of three. Adolescents’ median age was 16; five were girls and five were non-Hispanic Black. Most caregivers were mothers, five were non-Hispanic Black, and their median age was 41. All adolescents were willing to involve a caregiver in PrEP use. Participants said caregiver efforts to overcome cultural taboos made adolescents more willing to talk about sex. Caregivers described changing social norms and their own upbringing as reasons to support more open discussion. They also wanted to participate in conversations with health providers and support adolescents using PrEP. These qualitative findings come from a small sample and may apply only to some families. The authors concluded that caregiver-focused communication could be explored as one strategy to support PrEP among selected adolescents.
