The researchers worked with young Black men and clinic staff in Mississippi to design an HIV-prevention support program based on acceptance and commitment therapy. They wanted to adapt its content, delivery, and implementation to local barriers, values, and experiences.

Plain Language Abstract

Young Black men who have sex with men in Mississippi face high HIV incidence and substantial unmet need for pre-exposure prophylaxis, or PrEP. Researchers surveyed and interviewed 20 PrEP-eligible young men and 10 clinic staff from October 2021 through April 2022. They explored structural barriers, stigma, psychological flexibility, health behaviors, personal values, and preferences for adapting an acceptance and commitment therapy approach. Patients emphasized side effects, cost, and daily medication as barriers. Staff most often cited fear that others would assume a PrEP user had HIV. Interviews also examined emotions and associations related to HIV, existing coping, medication attitudes, values involving health, relationships, intimacy, and longevity, and practical adaptations. The findings informed ACTPrEP, a new intervention intended to help participants connect short-term discomfort with longer-term values and health goals. This was an intervention-development study. It did not deliver ACTPrEP or test whether it increased PrEP initiation, persistence, psychological flexibility, or HIV prevention outcomes.
