The researchers wanted to understand Tennessee primary care providers’ knowledge and attitudes about HIV-prevention medicine and whether they had prescribed it. They compared prescribers and nonprescribers to identify possible targets for future support.

Plain Language Abstract

Pre-exposure prophylaxis, or PrEP, is underused in the Southern United States despite high HIV prevalence. Researchers conducted an anonymous electronic survey of Tennessee primary care providers from March through November 2019. Of 147 responses, 99 met inclusion criteria, and 43 providers (43%) reported prescribing PrEP in the previous year. Prescribers had higher median knowledge scores than nonprescribers, 7.3 versus 5.6 out of eight. They were also more likely to report receiving patient questions about PrEP (95% versus 21%), feeling skilled at taking sexual histories from men who have sex with men (83% versus 58%), and feeling comfortable doing so (92% versus 63%). Overall, 65% felt obligated to provide PrEP, and 63% believed all primary care providers should offer it. The results identify knowledge, patient demand, and sexual-history skills as possible areas for future intervention. However, this cross-sectional survey cannot determine whether knowledge and comfort led to prescribing, whether prescribing increased those skills, or whether another factor influenced both. Respondents may not represent all Tennessee clinicians.
