The researchers compared a one-hour combined HIV-prevention and cultural-competence training with standard continuing education for 56 health care providers. They wanted to learn whether providers accepted the training and whether knowledge, confidence, intentions, or clinical behavior changed.

Plain Language Abstract

Provider knowledge and cultural responsiveness may affect equitable access to pre-exposure prophylaxis, or PrEP. Researchers conducted a pilot randomized controlled trial with 56 health care providers. One group received a one-hour session combining PrEP clinical content and cultural competence, while the comparison group received standard HIV continuing medical education. Participants rated the combined training favorably and reported greater PrEP knowledge. The intervention also increased confidence in performing PrEP-related clinical tasks and intention to prescribe. The proportion of providers discussing PrEP with patients rose only slightly in both groups. Actual PrEP prescribing and self-rated cultural competence did not change in either group. The findings show that a brief training was acceptable and influenced several self-reported intermediate outcomes, but they do not demonstrate improved prescribing or patient access. As a small pilot, the study was not designed to provide definitive evidence of clinical effectiveness. Larger studies with objective measures and longer follow-up would be needed to determine whether training changes routine care or reduces inequities.
