The researchers examined how three Philadelphia sexual health clinics were making HIV prevention medication easier to access. They asked clinic staff what was working, what resources were missing, and what changes could improve delivery.

Plain Language Abstract

Low-threshold pre-exposure prophylaxis, or PrEP, services are designed to reduce practical barriers to starting and continuing HIV prevention care. Researchers held three focus groups with 21 providers, prevention navigators, and administrative staff at three Philadelphia sexual health clinics from November 2021 to January 2022. They later interviewed eight staff members about successful strategies and needed adaptations. Staff created maps showing the steps from connecting clients to services through navigation, clinical care, and follow-up. Participants described co-locating and integrating services, strengthening staffing and clinic capacity, and directly addressing client barriers as important implementation strategies. The study used rapid qualitative analysis and an implementation framework to organize staff experiences. Because it focused on staff perspectives during early implementation, it did not estimate client outcomes or test whether the service model was more effective than standard PrEP care. The authors concluded that lessons from these clinics can guide continued local adaptation and future expansion, while additional evaluation is needed as the model scales.
