The researchers interviewed young men at different stages of PrEP use to understand what affected starting and taking it and whether a peer navigator could help.

Plain Language Abstract

Young men who have sex with men have high HIV incidence but use pre-exposure prophylaxis, or PrEP, less often than older adults. Researchers conducted 32 interviews at a Massachusetts health center specializing in sexual and gender minority care. Participants represented four groups: those who had never discussed PrEP with a provider, had discussed but declined it, were prescribed PrEP with low adherence, or were prescribed PrEP with good adherence. Participants described cost, expected stigma, sexual activity, and relationship status as influences on starting and continuing PrEP. Developing a regular pill-taking routine supported adherence. Many felt peer navigators could provide useful information, encouragement, and practical help. Because the study was qualitative, it describes themes across these participants and does not measure how often each barrier occurs or whether peer navigation improves adherence. The authors concluded that peer navigation may be a useful strategy for young men, but the approach should be designed around the different needs of people at each stage of PrEP engagement and tested directly.
