The researchers studied a drop-in HIV clinic for people experiencing homelessness or unstable housing. They wanted to learn how patients’ HIV levels, clinic engagement, and survival changed during the first year and whether baseline characteristics predicted very low HIV levels in the blood.

Plain Language Abstract

People experiencing homelessness often face structural barriers to regular HIV care and to reducing HIV in the blood to a very low level, known as viral suppression. Researchers followed 112 patients with unstable housing and unsuppressed HIV after enrollment in POP-UP, a drop-in San Francisco clinic offering integrated support without appointments. At enrollment, 52% were living outdoors, all had a substance use disorder, and 70% had a mental health diagnosis. After 12 months, 70% had attended at least one visit in every four-month period, although 59% had a care gap lasting 90 days. Forty-four percent had viral suppression, 24% remained unsuppressed, 23% had missing viral-load data, and 9% had died. Viral suppression rose from 0% at enrollment to 44%, but the observational design cannot establish that POP-UP alone caused this change. The study shows that a low-barrier model reached and retained some patients with substantial needs, while care gaps and overdose-related deaths remained serious challenges.
