Plain Language Abstract

People who inject drugs experience a disproportionate burden of HIV in the United States. Researchers used national data from 2008 through 2020 to describe changes and demographic disparities in new HIV diagnoses attributed to injection drug use. Over that period, diagnoses linked to injection drug use fell by 53.9%. Despite this decline, rates remained markedly unequal. New diagnosis rates per 100,000 people who inject drugs were 100.9 among women, 258.8 among Black people, and 131.0 among Hispanic people. The results show that overall progress did not eliminate disparities by sex or race and ethnicity. Prevention, testing, and treatment planning should therefore consider which populations continue to experience the highest diagnosis rates rather than relying only on the national downward trend. The brief abstract does not describe a tested intervention or establish why the disparities persisted.
